databooq is a form builder for surveys, quizzes, capacity-limited registrations and embeddable widgets. Everything happens in three places: the dashboard (your forms), the builder (one form’s questions, design, flow and settings) and the responses view.
Create a form
Click + Create form on the dashboard. Creation is three quick steps:
- Type: survey, quiz, registration or embed. Each opens a builder tuned to its job.
- Starting point: a grid of previews: Start blank (the + card) or any template. Built-in templates come first, then any you saved yourself, marked Yours. For embeds, this step is four layout presets to pick from.
- Name it: a title and optional description, pre-filled from a template and always editable.
To reuse a form you like, open its menu on the dashboard and choose Save as template; it then appears under Yours in the grid for any new form (webhook secrets and schedules are stripped out).
Build
Add questions by clicking or dragging types from the left palette. Edit labels and options directly on the card; fine-tune behavior (required, validation, images, seat limits) in the right inspector. Beyond the Build tab, the same top bar carries Design, Flow and a full Settings tab.
Publish and share
A form starts as a draft: respondents cannot see it. Publish it from the top bar or the Share page to make it live at its public link. Close it any time; reopen later. Responses arrive in the Responses tab in real time.
Autosave
The builder saves automatically a moment after every change. The chip in the top bar shows Saving… and Saved. If you edit the same form in two tabs, the older tab warns before overwriting newer changes.
